Double-J stenting during TURBT increases risk of metachronous upper tract urothelial carcinoma

The potential impact of double-J (DJ) stenting during transurethral resection of bladder tumor (TURBT) on the occurrence of metachronous upper urinary tract urothelial carcinoma (UTUC) remains a subject of debate. This study investigates the effect of DJ stenting during TURBT for bladder cancer (BC) on the incidence of metachronous UTUC.
MethodsWe retrospectively analyzed the data from patients who underwent TURBT at our institution between September 2013 and September 2023. Patients were grouped based on whether DJ stents were placed during TURBT. The outcome was the occurrence of metachronous UTUC, defined by UTUC occurring 3 months after TURBT. We define UTUC in DJ group occurring 3 months to 24 months post-TURBT as stent-related metachronous UTUC. Propensity scores adjustment was computed to control the confounding factors (p < 0.05). Kaplan-Meier analyses estimated metachronous UTUC recurrence-free survival (RFS) before and after propensity scores adjustment. Cox regression analyses were used to evaluate the data. Subgroup analyses and interaction tests for all categorical variables were conducted.
ResultsA total of 570 patients were included in this study. We identified 19 (3.33%) patients were metachronous UTUC. Among patients with metachronous UTUC, 7 patients received DJ stents, while 12 patients did not receive DJ stents. Kaplan-Meier analysis showed poorer recurrence-free survival in the DJ group during the first two years postoperatively (unadjusted p < 0.001 VS. adjusted p = 0.006) and throughout the entire follow-up period (unadjusted p < 0.001 VS. adjusted p < 0.001). Cox multivariable regression analysis revealed that the placement of DJ stent was an independent risk factor for metachronous UTUC-free survival rates within the first two years postoperatively ( p = 0.003) or during the overall follow-up period ( p < 0.001).
ConclusionsPatients who underwent DJ stenting during TURBT for BC exhibited an increased risk of developing metachronous UTUC, suggesting a potential association that warrants further investigation.
