A new device for lumbar intervertebral foramen lateral puncture: more accurate and less radiation exposure

The aim of this study was to explore the application value of a new puncture guiding device in lumbar lateral intervertebral foramen puncture. The new intervertebral foramen puncture guiding device based on the sector principle, first insert the guiding needle to the outer edge of the superior articular process, and then guide the puncture needle to directly intersect with the guiding needle at the superior articular process along the preset puncture point through the slide rail to complete the accurate puncture. From June 2020 to December 2022, 28 cases performed using the conventional method (Group A) and 30 cases utilizing the novel guidance method (Group B)0.58 cases of lumbar intervertebral foramen puncture under the guidance of guide frame or conventional method were completed. The operative time, the fluoroscopy times and the VAS scores from location to puncture was recorded. The operative time for lateral intervertebral foramen puncture was 14.17 ± 1.04 min in Group B, significant 18.41 ± 0.76 min in Group A (P < 0.05). Similarly, the mean fluoroscopy time was significantly lower in Group B (3.17 ± 0.53) than in Group A (7.18 ± 0.86) (P < 0.05). Postoperative VAS scores showed no statistically significant difference between the groups (P = 0.23). The designed guide frame has the characteristics of portability and X-ray transmission, which can significantly improve the accuracy of puncture, effectively shorten the learning time of lateral intervertebral foraminal endoscopic positioning puncture technology, reduce the incidence of puncture related complications and greatly reduce radiation damage.
