Purpose <p>To compare the functional and anatomical outcomes of 25-gauge pars plana vitrectomy (PPV) with and without uncomplicated cataract surgery for idiopathic epiretinal membrane (IERM) in phakic eyes.</p> Methods <p>In this retrospective clinical study, we recruited 85 phakic eyes with IERM and distributed in two groups: pars plana vitrectomy group (PPV group) and PPV combined with uncomplicated cataract surgery group (Combined group). Examinations were at baseline, 1 month, 3 months, 6 months and 12 months after each surgery. Primary outcome was the metamorphopsia; secondary outcomes were the best-corrected visual acuity (BCVA) and central macular thickness (CMT).</p> Results <p>The vertical and horizontal metamorphopsia scores decreased significantly in both groups at 12 months after surgery. There was no statistical difference between the two groups in vertical and horizontal metamorphopsia reduction at 12 months (<i>p</i> = 0.513 and 0.522 respectively). The mean BCVA of both groups improved significantly at 12 months after surgery (<i>p</i> = 0.003 for the PPV group, <i>p</i> &lt; 0.001 for the Combined group), the final BCVA in the Combined group was better than that in the PPV group (<i>p</i> = 0.011). The mean CMT also decreased significantly at 12 months after surgery in both groups (<i>p</i> &lt; 0.001 for both groups), there was no statistical difference between the two groups in CMT at 12 months (<i>p</i> = 0.160).</p> Conclusions <p>Combined vitrectomy with uncomplicated cataract surgery may potentially be as effective as vitrectomy alone with respect to metamorphopsia and anatomical outcomes for IERM patients. However, the BCVA recovery was better in the Combined group.</p>
