Background and aims <p><i>Helicobacter pylori (H. pylori)</i> is a frequent infection, with a higher prevalence in developing countries. French Guiana, although French, is a region with a high level of poverty and a high incidence of gastric cancer, for which <i>H. pylori</i> is a known risk factor. However, epidemiological data regarding <i>H. pylori</i> infection in this region remain scarce. The aim of this study was to assess the prevalence of <i>H. pylori</i> infection and resistance to clarithromycin.</p> Methods <p>We retrospectively included all patients who underwent a gastroscopy during which gastric biopsies samples were taken for molecular biology testing, <i>H. pylori</i> PCR. Culture was performed if <i>H. pylori</i> PCR detected resistance to clarithromycin. Sex, age and place of birth were analyzed to describe the population and the factors associated with infection.</p> Results <p>A total of 443 patients were included. <i>H. pylori</i> PCR was positive in 174 patients (39.3%). The median age of the whole population was 56 years (42–66). There was no significant difference between the proportions of men and women infected. Infection was less common in patients aged over 50 (<i>p</i> = 0.013) and was more frequent in people born abroad (<i>p</i> = 0.001). Being born in Haiti, Brazil or Suriname-Guyana was associated with a positive <i>H. pylori</i> PCR test (OR = 2.47, 95% CI [1.20–5.24], <i>p</i> = 0.02; OR = 4.26, 95% CI [1.94–9.67], <i>p</i> &lt; 0.001;OR = 4.40, 95% CI [1.70-11.76], <i>p</i> &lt; 0.001 respectively), while being over 50 years of age was associated with a negative <i>H. pylori</i> PCR test (OR = 0.33, 95% CI [0.15–0.69], <i>p</i> &lt; 0.001).Clarithromycin resistance was found in 20 (11.5%) of the infected patients. This was primary resistance in almost all cases (19/20 or 95%).</p> Conclusion <p>The prevalence of <i>H. pylori</i> infection on gastric biopsies is high in French Guiana, but resistance to clarithromycin is low, below the 15% threshold defined by the WHO. It is therefore important to establish a screening and treatment protocol tailored to the region.</p> Clinical Trial <p>Not applicable.</p>
