Preoperative hemoglobin level and its relationship with mortality and adverse outcomes in older hip fracture patients from Qatar

While several studies have shown an association between anemia severity and mortality in older hip fracture patients, its association with adverse outcomes such as delirium, postoperative complications, and prolonged length of stay is still unclear. Additionally, few data on this subject are available from the Middle East, which has an ethnically different and diverse population. We examined the associations between anemia at admission and outcomes in a cohort of older hip fracture patients from Qatar.
MethodsThis was a single-center observational cohort study of 322 hip fracture patients aged ≥ 60 years (range 60–100 years) from Qatar who were admitted with hip fracture and were followed up for four outcomes of interest: incident delirium, postoperative complications, one-year all-cause mortality, and increased length of stay (length of stay ≥ 10 days). Multivariable-adjusted logistic regression models were used to calculate odds ratios and 95% confidence intervals (OR [95% CI]) and p-trend values were derived using ordinal coding in corresponding logistic regression models.
ResultsThree hundred and twenty-two hip fracture patients, with an average age of 76.8 years, were included in our study. At baseline, 32.6% had no anemia, 31.4% had mild anemia, and 36.0% had moderate/severe anemia. Increasing anemia severity was positively associated with incident delirium, postoperative complications, increased length of stay, and all-cause mortality. Compared with no anemia (referent), the multivariable OR (95% CI) for those with moderate/severe anemia was 2.44 (1.07–5.57), p-trend = 0.03 for delirium, 4.07 (1.63–10.19); p-trend = 0.001 for postoperative complications, 2.12 (1.21–3.71); p-trend = 0.01 for length of stay ≥ 10 days, and 3.79 (1.18–12.20); p = trend = 0.02 for all-cause mortality.
ConclusionAnemia at admission was positively associated with delirium, postoperative complications, length of hospital stay and all-cause mortality in hip fracture patients from Qatar.
