<p>Guillain-Barré Syndrome (GBS) is an acute, immune-mediated polyradiculoneuropathy, with several clinical variants including the acute motor axonal neuropathy (AMAN) subtype. AMAN often presents with rapid motor weakness, respiratory compromise, and may follow antecedent infections. We report a rare and severe case of AMAN in a 25-year-old female presenting with rapidly progressive quadriplegia and type II respiratory failure. She required prolonged mechanical ventilation, developed multiple hospital-acquired infections, and experienced infusion-related adverse reactions to intravenous immunoglobulin (IVIg). Despite these complications, gradual improvement was achieved through multidisciplinary care. This case underscores the potential severity of AMAN, especially when complicated by immunotherapy intolerance and secondary infections. It highlights the importance of early diagnosis, vigilant respiratory support, and individualised treatment strategies in improving outcomes in severe GBS variants.</p>
