Abstract <p>The aim of this study was to evaluate the clinical efficacy and safety of moxibustion combined with acupressure in the treatment of refractory hiccups after stroke. Forty-five stroke patients with dysphagia and refractory hiccups were retrospectively designated as the control group. Another 45 patients with the identical condition who received traditional Chinese medicine rehabilitation care combined with care bundles in the same department were designated as the observation group. The differences in swallowing function, hiccup scores, cerebral blood flow parameters, inflammatory markers, autonomic nervous system function, and quality of life scores before and after intervention, as well as the incidence of complications were compared between the two groups. At both 2 and 4 weeks after intervention, the SSA scores and hiccup scores of the observation group were significantly lower than those of the control group (<i>P</i> &lt; 0.05). At 4 weeks after intervention, the observation group exhibited higher mean blood flow velocity, standard deviation of heart rate variability, and quality of life scores, while the pulsatility index, low-frequency/high-frequency ratio, lnterleukin-6 (IL-6), tumour necrosis factor alpha (TNF-α), and C-reactive protein (CRP) levels were significantly lower than those in the control group (<i>P</i> &lt; 0.05). Additionally, the incidence of adverse reactions was significantly lower in the observation group compared to the control group (8.89% vs. 24.44%) (<i>P</i> &lt; 0.05). The application of moxibustion combined with acupressure for patients with refractory hiccups after stroke is undoubtedly valuable.</p>
