<p>In prostate cancer treatment, nerve-sparing surgery can preserve erectile function, but which components of sexual function men most value, and how sexual function is actually lost with age, remain unclear. We conducted a cross-sectional web-based survey of 6,000 Japanese men aged 50 years or older, who were asked to respond to a hypothetical scenario of receiving prostate cancer treatment. Participants ranked five sexual and reproductive domains (ejaculation, erection, orgasm, sexual arousal, and fertility) by how much they least wanted to lose, and reported the current status of each function. Sexual arousal was most often ranked the function least wanted to lose (42.8%), followed by erection (26.0%). The importance of erection increased with age (<i>p</i> = 0.002), whereas the other functions were stable. Complete loss increased with age; by 80 to 84 years it was highest for ejaculation (53.5%) and lowest for sexual arousal (29.0%), but decline began earliest for erection. Erection was valued more as its status worsened (<i>p</i> = 0.007), whereas orgasm was valued less as it was lost (<i>p</i> &lt; 0.001). In older Japanese men, sexual arousal was most valued, and the importance of erection rose as it declined, in contrast to orgasm. Because these data are cross-sectional and were obtained under a hypothetical scenario in an online panel, they reflect attitudes rather than real-time treatment preferences, and the mechanisms remain hypothetical. These findings may inform counselling that considers age and current function, not erection alone.</p>
