Low-carbohydrate Mediterranean style diet improves pain and body composition in women with lipedema: role of redox balance

Lipedema is a chronic adipose tissue disorder characterized by disproportionate fat accumulation, pain, tenderness, and impaired physical function. Although dietary interventions may alleviate symptoms, the mechanisms underlying these effects remain unclear. This study evaluated the effects of an energy-restricted low-carbohydrate high-fat Mediterranean-style diet on systemic redox balance and pain intensity in women with lipedema. Women with lipedema (n = 24) and women with overweight/obesity without lipedema (n = 24) followed the dietary intervention for 7 months. Systemic oxidative stress and antioxidant biomarkers were assessed before and after the intervention, and their associations with adiposity, inflammatory markers and pain intensity were analyzed. At baseline, women with lipedema had lower protein carbonyl concentrations and higher total antioxidant capacity than controls, whereas other redox and inflammatory markers were comparable. After the intervention, both groups showed significant reductions in body weight and adiposity. In women with lipedema, the diet reduced lipid peroxidation and protein carbonyl concentrations, improved antioxidant status, and markedly decreased pain intensity. Pain reduction was independently associated with decreased TBARS concentrations, whereas changes in inflammatory markers were not independently associated with pain improvement. These findings suggest that improvements in systemic redox balance may contribute to pain relief in women with lipedema. A low-carbohydrate high-fat Mediterranean-style diet may therefore represent a promising non-pharmacological approach to symptom management in lipedema.
