<p>Cataracts are primarily age-related; however, factors associated with earlier progression to surgery among working age adults remain unclear. Identifying modifiable lifestyle factors may provide insights into delaying surgical intervention. This study used the Japanese Medical Data Center Claims Database (January 2005 to August 2023). Patients who underwent cataract surgery (n = 30,971) were identified as cases. Controls (n = 92,913) without cataract-related diagnoses or surgery were matched 1:3 on age at health screening, sex, and screening year. Conditional logistic regression using linked health screening data was performed to estimate odds ratios (ORs) and 95% confidence intervals (CIs). Subgroup analyses by age group were also conducted. Hypertension (OR, 1.10; 95% CI, 1.06–1.13), diabetes mellitus (OR, 1.97; 95% CI, 1.89–2.06), renal dysfunction (OR, 1.12; 95% CI, 1.06–1.18), hyperuricemia (OR, 1.07; 95% CI, 1.01–1.14), and excessive alcohol consumption (OR, 1.15; 95% CI, 1.10–1.21) were significantly associated with cataract surgery. Age-stratified analyses showed that ORs for hypertension and diabetes mellitus generally decreased with increasing age, and interaction tests were also statistically significant. These findings indicate that modifiable lifestyle-related conditions were associated with earlier progression to cataract surgery in working age adults.</p>
