<p>Diphtheria is an infectious disease caused by the bacterium Corynebacterium diphtheriae, which can be prevented by vaccination. Currently, there are no comprehensive data on the demographic and temporal patterns of diphtheria in Abyan Governorate. Accordingly, this epidemiological analysis aimed to highlight the overlooked diphtheria burden in the Abyan Governorate, Yemen, over five years from 2020 to 2024. This registry-based surveillance analysis relied on secondary data on diphtheria cases registered in the official electronic database in the study area. The required data were electronically gathered and analyzed using Excel and SPSS programs. A total of 547 diphtheria cases were reported in the Abyan Governorate, yielding a cumulative incidence of 88.0 cases per 100,000 inhabitants. Most diphtheria cases were found in females, with 296 (54.1%) and a cumulative incidence of 96.6 cases per 100,000; in those aged 5–9 years, with 192 (35.1%) and a cumulative incidence of 272.4; in Lawdar District, with 378 (69.1%) and a cumulative incidence of 296.2; as well as in 2020, with 133 (24.3%) and an annual incidence of 22.0 cases per 100,000 person-years. Additionally, the majority of cases (92.1%) were unvaccinated. These findings revealed statistical significance in the distribution of cases across all tested variables, including age groups (P = 0.019), sex (P &lt; 0.001), annual factors (P &lt; 0.001), and geographic factors (P &lt; 0.001). The overall case fatality rate was reported to be 8.04% (44 cases), with a higher rate among the 5–9 age group (11.5%); in 2021 (14.3%); and in Jayshan District (50.0%), as well as all deaths being recorded among unvaccinated individuals (8.7%). These data indicate that the annual diphtheria pattern emerged as a fluctuating trend in this study area. Therefore, health institutions should implement strategies based on expanding vaccination coverage, increasing public awareness, strengthening surveillance systems, and improving health systems capable of early detection and rapid response, as well as building multisectoral partnerships to combat and control diphtheria epidemics. </p>
