Mpox knowledge levels and associated factors among MSM in Chongqing and Chengdu, China

Monkeypox (Mpox) has become a major public health threat globally, with men who have sex with men (MSM) being the primary affected population. In China, the mpox epidemic has spread to multiple provinces, but regional research on MSM in the Chongqing-Chengdu urban agglomeration is lacking. A cross-sectional study was conducted among MSM in Chongqing and Chengdu from August to December 2024 using convenience sampling via community-based organizations. A self-designed questionnaire was used to collect data on socio-demographic characteristics, sexual behaviors, HIV-related status, mpox information attention and knowledge level. Multivariable logistic regression was used to identify factors associated with mpox knowledge awareness. A total of 514 valid questionnaires were collected. The overall awareness rate of mpox knowledge was 49.6%, with a median knowledge score of 9.0 (IQR: 6.0, 12.0). Participants demonstrated high accuracy in responses related to preventive measures (82.1%) and sexual transmission routes (80.0%), whereas correct responses were lower for the incubation period (53.5%) and universal susceptibility (56.6%). The primary source of mpox information was the internet and social media (49.8%). Multivariable analysis showed that having a college education or higher (aOR = 2.47, 95% CI 1.32–4.60), identifying as homosexual (aOR = 1.57, 95% CI 1.01–2.44), absence of group sex in the past year (aOR = 2.55, 95% CI 1.09–5.93), travel to mpox-endemic countries in the past year (aOR = 4.18, 95% CI 1.32–4.60) and actively seeking mpox-related information (aOR = 3.40, 95% CI 2.03–5.71) were independent factors associated with higher mpox knowledge levels. The level of mpox knowledge awareness among MSM in Chongqing and Chengdu was moderate, characterized by an uneven distribution of knowledge. Information acquisition was predominantly through digital channels with insufficient professional input. Targeted health education strategies should be developed for key populations, and an integrated promotion model of “professional personnel + authoritative new media” should be constructed to improve the effectiveness of mpox prevention and control in this population.
