CA-125/albumin and lymphocyte/albumin ratios as complementary biomarkers to the malignancy risk index in ovarian cancer

The aim of this study was to evaluate the usability of the lymphocyte/albumin (L/A) ratio as a potential biomarker in determining the risk of malignancy in ovarian cancer and to make a comparative analysis of this ratio with the Risk of Malignancy Index (RMI). Designed as a retrospective study, this study was conducted with 130 patients who underwent surgery for ovarian mass between 2020 and 2023. Demographic, clinical and laboratory data (CA-125, albumin, lymphocytes, neutrophils, monocytes) of the patients were collected retrospectively. L/A ratio, CA-125/albumin, neutrophil/lymphocyte (N/L) and monocyte/lymphocyte (M/L) ratios were calculated. RMI was calculated using ultrasound findings, menopausal status and CA-125 levels. Statistical analyses were performed with IBM SPSS 27 package program and diagnostic accuracy was evaluated using ROC curve analysis. RMI, CA-125, monocyte, neutrophil, CA-125/albumin, N/L and M/L values were significantly higher in the malignant patient group than in the benign patient group (p < 0.05). The L/A ratio was lower in the malignant patient group (p = 0.035). The CA-125/albumin ratio had the highest diagnostic accuracy (AUC = 0.13, p < 0.001). The AUC value of the L/A ratio was calculated as 0.61. This study showed that L/A ratio can be used as a potential biomarker in determining the risk of malignancy in ovarian cancer. Especially the high diagnostic accuracy of CA-125/albumin ratio suggests that this combination should be evaluated as a complementary method to RMI. Larger-scale studies are needed to elucidate the role of inflammatory biomarkers in the diagnosis of ovarian cancer.
