<p>NHHR is the new composite lipid index. There are few studies on the relationship between NHHR and bone mineral density (BMD). This study aimed to investigate the correlation between the NHHR and BMD. Weighted multiple linear regression, threshold effects, and fitted smoothed-curve modeling were employed in this cross-sectional analysis, which comprised 3,138 participants in the 2011–2018 NHANES, to examine the relationship between NHHR and BMD. A total of 3,138 participants were analyzed and grouped by NHHR quartiles. Higher NHHR was associated with male gender, younger age, and increased cardiometabolic risk factors. NHHR was negatively correlated with BMD, and a significant non-linear relationship was observed, with an inflection point at 1.301. Below this threshold, NHHR was significantly inversely associated with BMD. Subgroup analyses revealed stronger associations in participants &lt; 20 years and females. These findings suggest NHHR may be an independent biomarker for BMD, particularly in youth and women, offering insights into lipid–bone metabolism cross-talk. A U-shaped connection between NHHR and BMD was found in this study. As a novel lipid marker, NHHR can help diagnose osteoporosis early and direct treatment.</p>
