<p><i>Pseudomonas aeruginosa</i> (<i>PA</i>) bloodstream infections (BSIs) are severe, life-threatening events. <i>PA</i> success involves a complex interplay between virulence, antimicrobial resistance and epidemicity. This study examines the impact of COVID-19 pandemic on <i>PA</i>-BSI characteristics. Seventy-eight bacteraemic <i>PA</i> isolates were collected in two periods: pre-COVID 2015–2018 (48 <i>PA</i>) and post-COVID 2022–2023 (30 <i>PA</i>). An increase in non-susceptibility rates was found in post-COVID period, noteworthy for gentamicin, netilmicin and multidrug-resistance, although extensively drug resistant/difficult-to-treat (XDR/DTR) isolates were only detected in pre-COVID period. All isolates were cefiderocol-susceptible, but resistance to novel cephalosporin-inhibitor combinations and colistin was identified in pre-COVID period. Carbapenem-non-susceptible <i>PA</i> persisted around 20%, associated with OprD alterations and one pre-COVID-isolate harboured <i>bla</i><sub>VIM-2</sub>. High clonal diversity was found: 71 pulsotypes and 52 sequence types (most prevalent: ST17, ST244 and ST274). A shift in serotypes was detected between pre-COVID-<i>PA</i> (O:1) and post-COVID-<i>PA</i> (O:6). Eight virulotypes were detected, highlighting one <i>exlA</i>-hypervirulent post-COVID-<i>PA</i>, and <i>exoU</i><sup>+</sup><i>/exoS</i><sup>-</sup> genotype in 17% pre-COVID-<i>PA</i> but none post-COVID-<i>PA</i>. Biofilm production was significantly higher in pre-COVID-<i>PA</i>. This work shed light on the evolving nature of <i>PA</i> infections amidst the COVID-19 pandemic, providing valuable insights for clinical management and reinforcing the importance of judicious antibiotic use in the context of public health crises.</p>
