<p>The aim of this review was to characterise the clinical features of basal cell carcinomas (BCCs) occurring on the lips. A review of the literature was performed using PubMed to identify reports of BCCs of the lips. Additionally, a retrospective search of the archives of our diagnostic oral pathology service was performed for BCCs diagnosed between 2000-2023. There were 27 BCCs of the lips identified in the literature review. These tumours typically presented clinically as bleeding, eroded, crusted, ulcerated and/or centrally depressed nodules. Treatment included surgical excision, Mohs micrographic surgery, wedge resection and radiation. None recurred or metastasised. Ten additional cases of BCC of the lips were retrieved from our archives. Six of these BCCs exhibited a classic clinical presentation, being described as red, white, crusted, bleeding, scabbing, eroded and/or ulcerated lesions. Four tumours were considered non-classic, presenting as yellow nodules. To the best of our knowledge, this is the first report of BCCs on the lips presenting as yellow nodules. It is important for clinicians to be aware of this rare presentation of BCC in the oral and perioral region as it may mimic benign entities.</p>
