Prosthetic rehabilitation in patients diagnosed with stage IV periodontitis requires a structured approach. Comprehensive clinical examination and assessment of each tooth in terms of its prognosis, its restorability, and the value of its retention is needed in order to establish a preliminary treatment plan. The initial phase of treatment aims to improve the patient’s oral hygiene, with periodontitis treatment and caries restoration to achieve the absence of inflammation. Teeth with a good prognosis can be planned as prosthetic abutments. Teeth with a questionable prognosis—including those with severely reduced residual attachment—are reevaluated after the hygiene phase. Teeth with a poor prognosis (“irrational to treat”) are extracted. During the reevaluation, decisions can also be made regarding further surgical or orthodontic interventions. Definitive prosthetic restoration is performed in the reconstructive phase, and the patient transitions into the maintenance phase. For long-term success and preservation of periodontal stability, supportive periodontal therapy (SPT) is essential.
