Background <p>The most serious complications of laparoscopic sleeve gastrectomy (LSG) are staple line leakage and bleeding. This study aimed to retrospectively determine whether oversewing of the staple line had an effect on postoperative bleeding and leakage in patients undergoing sleeve gastrectomy.</p> Methods <p>This retrospective study was conducted at a single center with approval from the ……. ……. A total of 2541 patients aged 18–65 years who underwent sleeve gastrectomy at a single center between January 1, 2012, and December 31, 2024, were included in the study. The patients included in the study were defined as Group 1 (Stapler Line Non-Suturing) and Group 2 (Stapler Line Suturing) based on whether oversewing of the staple line was performed.</p> Results <p>A total of 2541 patients were included in the study, with 456 patients in Group 1 and 2085 patients in Group 2. Preoperative and postoperative 1, 3, 6, and 12-month BMI values were statistically significantly higher in Group 1 (<b>p &lt; 0.001</b>). From the preoperative period to the first year postoperatively, Group 2 showed significantly greater weight loss compared to Group 1, except for the preoperative-postoperative 1/3 months and postoperative 1-postoperative 3 months (<b>p &lt; 0.001</b>, preoperative-postoperative 6 months; <i>p</i> = 0.010). Bleeding, leakage, and total complication rates were statistically significantly higher in Group 1 (respectively; <i>p</i> = 0.004,<b> p = 0.020</b>, and <i>p</i> = 0.005).</p> Conclusion <p>The results of our study, with a significantly larger sample size than studies in the literature, provide new evidence that SLS (Group 2) significantly reduces the risk of postoperative bleeding and leakage compared to Group 1. Furthermore, SLS (Group 2) may be associated with better weight loss compared to Group 1, consistent with the results of the limited number of studies in the literature.</p>

Background
