Ketogenic Diet Impacts Epilepsy via Alterations in Gut Microbiome Composition: A Review

Epilepsy is a neurological disorder characterized by recurrent seizures, and traditional treatment methods often yield variable outcomes. Recent investigations suggest that dietary interventions, particularly the ketogenic diet (KD), may play a significant role in epilepsy management. This review aims to elucidate the role of the ketogenic diet in influencing the gut microbiome and its subsequent impact on epilepsy control. A comprehensive literature search was conducted to identify studies exploring the relationship between the ketogenic diet, gut microbiome composition, and epilepsy outcomes. Relevant animal and human studies were analyzed to assess how the KD alters gut microbiota and how these changes correlate with the frequency and severity of seizures. The review identified a significant alteration in gut microbiome composition associated with the ketogenic diet. Key findings indicated increases in beneficial microbial taxa such as Bifidobacterium and Akkermansia, alongside decreases in potentially harmful taxa like Escherichia and Enterococcus. These microbiome changes were linked to enhanced metabolic profiles, reduced inflammation, and improved seizure control in both preclinical models and clinical settings. The ketogenic diet appears to exert its antiepileptic effects not only through metabolic pathways but also by modulating gut microbiome composition. This review highlights the intricate relationship between diet, gut health, and epilepsy, suggesting that future therapeutic strategies could benefit from considering microbiome-targeted approaches alongside traditional dietary interventions. Further research is warranted to explore these mechanisms and their implications for comprehensive epilepsy treatment.
