Vitamin C Augments the Protective Effects of Metformin on Diabetes-Induced Renal and Cardiac Complications Associated with the Inhibition of Oxidative and Nitrosative Stress and Dyslipidaemia As Well As Ameliorating Inflammation and Apoptosis

We sought to determine whether vitamin C can augment the protective effects of the anti-diabetic drug metformin against diabetes-induce renal and cardiac complications in an animal model of type 2 diabetes mellitus.
MethodsTwenty-five male albino rats were stratified into five groups (5 rats each). Groups 1 (Control) and 2 (diabetic) received rat diets and water; Groups 3, 4 and 5 were diabetic rats that received metformin (100 mg/kg), vitamin C (25 mg/kg) and metformin (100 mg/kg) plus vitamin C (25 mg/kg) respectively. The experiment took 6 weeks.
ResultsThe diabetic rats had marked elevation (P < 0.05) of fasting blood glucose, homeostatic model assessment of insulin resistance, relative kidney weight, urea, creatinine, uric acid, lactate dehydrogenase, creatine kinase-MB, aspartate amino transaminase activity but significant decrease in serum insulin, homeostatic model assessment of pancreatic beta cell function, dyslipidaemia, body weights and serum total proteins which resulted to oxidative stress, inflammation and apoptosis of their renal and cardiac tissues. Most of these changes were improved following treatment with metformin, vitamin C and their combination as was also affirmed by their kidney and heart histology. The modulatory potentials of vitamin C on renal and cardiac complications in the diabetic rats was akin to metformin in the current study.
ConclusionMetformin and vitamin C polytherapy was better than metformin in alleviating dyslipidaemia, renal and cardiac oxidative stress, inflammation and in improving histological changes in the kidneys and hearts of the diabetic rats.
