Purpose <p>To compare the real-life performance of three monofocal plus intraocular lenses (IOLs).</p> Methods <p>In this retrospective, single center, single surgeon case series, bilateral cataract patients with less than 1 diopter (D) of corneal astigmatism and no ocular comorbidities were included. Examinations were performed at 3&#xa0;months postoperatively and included monocular and binocular uncorrected and corrected distance visual acuity, and distance-corrected (DC) visual acuity at 80, 60 and 40&#xa0;cm, DC defocus curve with calculations of the area under the curve (AUC), contrast sensitivity and CATQUEST-9SF questionnaire. Results were all obtained with validated digital testing tablet applications.</p> Results <p>Among the 64 implanted eyes (32 patients) – 20 with Isopure123 (BVI Medical), 26 with Tecnis Eyhance (Johnson &amp; Johnson Vision) and 18 with RayOne EMV (Rayner) IOLs – statistically significant differences (p ≤ 0.05) were found regarding 3 post-surgery CATQUEST-9SF categories and the defocus curve: between Eyhance and RayOne EMV (binocular) at −1.50 and −1.00D of defocus and in the near AUC segment, and between Isopure123 and RayOne EMV (monocular) for 0.00 and + 1.00D of defocus and in the far distance AUC segment. No statistically significant differences were found between the 3 IOLs for monocular and binocular uncorrected and corrected visual acuity at all distances tested.</p> Conclusions <p>All 3 IOL models provided comparable visual benefits and good performance at intermediate and near distance. A bigger sample size might detect more differences, as some trends were visible, such as differences in the defocus curve and patient-reported outcomes.</p>
