Novel therapeutic vaccine development strategies targeting high risk HPV16/18-E6/7 oncoproteins, their immunogenicity, and efficacy associated with cervical intraepithelial neoplasia and cervical cancer: a systematic review

The high-risk human papillomavirus 16/18 (HR HPV 16/18) is substantial causes of high morbidity and mortality among women. Already infected women are still suffering from cervical cancer and cervical intraepithelial neoplasia (CIN) due to the absence of a therapeutic vaccine. The most potent therapeutic vaccine is expected to induce both cellular and humoral immunity.
ObjectiveThe aim of this review is to systematically assess novel therapeutic vaccine development strategies targeting high-risk HPV16/18-E6/7 oncoproteins, their immunogenicity, and efficacy associated with cervical cancer and CIN.
MethodsArticles were systematically searched in Scopus, PubMed, Hinari, and Google Scholar from 2014 to 2024. Only original articles conducted in human clinical trials were included. The Joanna Briggs Institute (JBI) Critical Appraisal Tools for Systematic Reviews Checklist for Systematic Reviews and Research Syntheses (http://joannabriggs.org/research/critical-appraisal-tools.html) were used to assess risk of bias in included studies. Five authors worked independently to avoid risks of bias. The data are presented in the form of tables and narratives. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA, 2020) guidelines were used to select studies.
ResultsOut of 555 pertinent studies searched, only 22 articles were included. Main outcomes observed in almost all enrolled patients were histopathological waning from high grade CIN to low grade, generation of HR HPV 16/18-specific humoral and cellular immune responses, viral load clearance, tumor regression, and virus-specific interferon and interleukin. Therapeutic vaccines administered with an adjuvant showed higher immunogenicity and efficacy than those without an adjuvant. Moreover, different strategies of HR HPV 16/18 vaccine development demonstrated varying immunogenicity and efficacy against cervical cancer and CIN.
DiscussionThe development of novel HR HPV 16/18 therapeutic vaccine strategies, their immunogenicity, and efficacy is very promising for cervical cancer and CIN therapy. The novel HR HPV 16/18 Therapeutic vaccines need to include different immune targets and adjuvants. Further studies are required to develop an effective HR HPV 16/18 therapeutic vaccine strategy, boost their immunogenicity and efficacy.
