<p>Non-tuberculous mycobacteria (NTM) infections are often misdiagnosed as spinal tuberculosis because some of their symptoms are similar to spinal tuberculosis, are rare in clinical practice, and are under-recognized, which seriously affects the treatment and prognosis of patients. <i>Mycobacterium xenopi</i> (<i>M. xenopi</i>) is a type of NTM, the incidence of which has increased in recent years. Here, we report on a 63-year-old female patient with spinal <i>M. xenopi</i> infection, focusing on the disease process and clinical treatment options and elaborating on spinal <i>M. xenopi</i> infection in the context of clinical features, imaging and laboratories, mNGS tests, management treatments, and the literature. The presentation of this report focuses on helping early clinical diagnosis of spinal <i>M. xenopi</i> infection and sharing clinical experience for subsequent treatment.</p>
