<p>The need for new therapeutic targets, biomarkers, and drugs for the treatment of liver injury, obesity, and hepatocellular cancer is critical due to the alarming rate of co-morbidity and mortality. Accordingly, the hepatokine fibroblast growth factor 21 (FGF21) is a promising candidate because it protects liver functions and regulates satiety. Furthermore, given that FGF21 is an endocrine hormone, it also maintains homeostasis of several metabolic pathways, making delivery of FGF21 an attractive therapy for protecting the liver against obesity-induced injury and carcinogenesis. However, molecular mechanisms responsible for several of the beneficial FGF21 functions are unclear, and detrimental properties of FGF21 have been reported, bringing into question the therapeutic value of FGF21. Thus, elucidation of the underlying molecular mechanisms that FGF21 regulates for protection against liver injury and hepatocellular cancer, and obesity resistance requires further understanding.</p>
