<p>Acute pancreatitis in the setting of traumatic diaphragmatic hernia is a very rare condition and very few cases have been reported in literature. In this paper we report of a 54-year-old truck driver who had a blunt abdominal trauma 2&#xa0;months prior and sought consultation at our emergency department due to vague abdominal pain with associated anorexia and vomiting. Serum lipase was found to be four times elevated. Radiologic studies done then revealed a diaphragmatic hernia without evidence of pancreatic herniation. This patient then underwent emergency thoracotomy to reduce the hernia which subsequently resulted in the resolution of the pancreatitis. Patient was discharged well on post-operative day 6. Traumatic diaphragmatic hernia presents a very rare cause of acute pancreatitis. Vigilance and thoroughness in the history and physical examination are critical in these cases. Because of the possibility complications, traumatic diaphragmatic hernia should promptly be treated once it is identified.</p>
