Gastric trichobezoar as first presentation of psychiatric disorders in young women: case series

Trichobezoars are rare hair masses that can cause abdominal symptoms and obstruction, commonly seen in children, adolescents, and adults with conditions like trichotillomania and trichophagia. Diagnosis often involves CT scans, and surgical removal is the primary treatment due to limited success with endoscopy. Post-surgery, psychiatric evaluation, and cognitive-behavioral therapy are essential to prevent recurrence. The condition is very rare in Sudan, and there is very scarce previous literature on the issue.
Methods and patientsThe series presented three gastric Trichobezoar cases. The patients did not have any prior psychiatric disorders, and they consented to participation and publication. Following clinical assessment, basic investigations, abdominal ultrasound, and upper gastrointestinal endoscopy were conducted to confirm the diagnosis. Subsequently, the patients underwent laparotomy and gastrotomy for the removal of the sizeable gastric bezoar. Postoperatively, they received nutritional support and were referred to the psychiatric department for counseling and future follow-up.
ConclusionTrichobezoar management requires a multidisciplinary approach that includes surgical intervention, nutritional support, and psychiatric evaluation. Early diagnosis through imaging and endoscopy is crucial for effective treatment, while addressing the psychiatric aspects is vital to prevent recurrence.
