<p>Idiopathic inflammatory myopathies (IIM) are rare but potentially life-threatening diseases. An interdisciplinary evaluation and management should always be performed. There are hardly any approved drugs, and a&#xa0;significant proportion of patients do not respond adequately to conventional therapies. B‑cell-targeted therapies—currently rituximab is most commonly used—have become established in rheumatology and the treatment of IIM as effective and generally well-tolerated options. Their use is particularly relevant in myositis-associated lung involvement and/or therapy-resistant myositis. Today, new targeted anti-CD20, anti-CD19, or anti-CD38 antibodies are available in immunology and are currently also being used experimentally in IIM. In this case report, we present a&#xa0;patient with therapy-refractory myositis with Jo‑1 syndrome and discuss modern B‑cell-targeted therapies in the context of the available literature.</p>
