Polymyalgia rheumatica (PMR) is a common inflammatory rheumatic disease that affects elderly individuals and is characterized by pain in the shoulder and pelvic girdle. The diagnosis is primarily clinical; however, imaging techniques can enhance the specificity of the diagnosis. In clinical practice, ultrasonography is particularly useful due to its good availability and ease of use. For detecting inflammatory pathologies in the pelvic girdle, MRI offers advantages, and in cases of atypical presentation, a PET-CT can be helpful for screening for large-vessel vasculitis or malignancy.
