<p>Patient activation describes the willingness to engage in health-related actions, self-efficacy refers to belief in one’s own abilities, and self-management represents the actual behavior they perform. All three concepts reinforce each other and promote personal responsibility and play an active role in managing illness. Those with chronic or multimorbid conditions particularly benefit from this. In general practice, motivational interviewing, SMART (specific, measurable, achievable, realistic, time-based) goals, and shared decision-making support the promotion of these concepts. Activation must be individualized and tailored to daily life, resources, and possibilities. Successful implementation improves physical and mental health as well as the quality of life.</p>
