Objectives <p>This study investigated factors influencing Brazilian dentists’ adoption of intraoral digital radiography, examining their perceptions and implementation challenges.</p> Methods <p>A cross-sectional study was conducted using semi-structured questionnaires distributed to registered dentists via Google Forms. Analysis included multinomial logistic regression to identify adoption factors and Kruskal-Wallis tests for Likert scale responses (significance level: 5%).</p> Results <p>Among 63 respondents, 73% performed intraoral radiographs in their clinics, but only 21% used digital systems. Major adoption barriers included high acquisition and maintenance costs (40%) and inadequate equipment (30%). Significant associations emerged between digital radiography use and patient income, frequency of radiograph usage, and institutional type (<i>p</i> &lt; 0.001). Digital system users reported significantly higher satisfaction levels (<i>p</i> &lt; 0.05).</p> Conclusions <p>Intraoral digital radiography adoption in Brazilian dentistry may be influenced by economic and practical factors. Financial constraints appear to represent the primary barrier, potentially contributing to a socioeconomic divide in technology access. While dental professionals recognize the clinical benefits of digital systems, substantial initial investment and maintenance costs may present challenges, especially for lower-income practices.</p>
