In the treatment of class III malocclusion, the optimal time for treatment is dependent on various factors, including the severity of the malocclusion, skeletal maturity stage, and individual growth prognosis. Due to progression of this malocclusion, early intervention is recommended, ideally during the early mixed dentition stage. The aim of this early treatment approach is to use the potential of craniofacial growth to avoid the need for future surgical correction. The therapeutic spectrum includes functional measures and complex appliance interventions. In the early phase and in cases of mild malocclusion, myofunctional exercises and removable appliances are particularly useful. In moderate to severe cases, the use of fixed and extraoral appliances may be indicated. In the later stages, orthognathic surgery is often the only option left.
