<p>The S3 guideline ‘Pit and fissure sealing' (PFS) considered for the first time in its current revision aspects of PFS in children and adolescents with special health care needs. The aim of this article is to present the current state of knowledge on PFS in this vulnerable patient group. To answer the agreed key question whether PFS leads to a caries reduction in children and adolescents with disabilities and special health care needs, the literature was systematically searched and analysed in detail. As a result, five reviews, eight cross-sectional studies, one (randomised) clinical trial and one cohort study that met the inclusion criteria were identified. Despite the small number of available studies, the existing heterogeneity and the overall low level of evidence, there was a strong consensus that PFS should generally be used in children and adolescents with special health care needs. Due to its increased caries risk, PFS should be used in particular for patients with mental and multiple severe disabilities. However, the current limited amount of data precludes a stronger level of recommendation and more detailed advice on the clinical procedure. Therefore, targeted study projects are required in this vulnerable patient group in order to increase the database.</p>
