Caries and in particular deep caries with involvement of the pulp in deciduous teeth is still frequently a serious problem in paediatric dentistry, despite all endeavors for prophylaxis. In modern endodontics a plethora of treatment options are available for clinically active paediatric dental specialists. Those options that preferably aim to preserve the vitality of the complete pulp of deciduous teeth have the best prospects. Among the many treatment recommendations available, those of the European Academy of Paediatric Dentistry (EAPD) are particularly emphasized and these differentiate two large groups: the classical pulp-preserving measures (pulpotomy and indirect pulp capping) and minimal intervention dentistry (repeated use of silver diamine fluoride, the Hall technique and selective caries reduction). Furthermore, there is the option of direct pulp capping. Deciduous teeth with irreversible pulpitis, abscesses or fistulas are no longer restorable and must nearly always be extracted.
