Purpose <p>Cadaver surgical training (CST) is central to off-the-job training in head and neck surgery, but access to expert tutors and didactic materials can be limited by cost, logistics, and biohazard constraints. Augmented reality (AR) may allow trainees to access stepwise guidance and 3D anatomy while preserving cadaveric haptics. This proof-of-concept study aimed to develop an AR-enhanced cadaver surgical training (AR-CST) system and evaluate its perceived usefulness and feasibility.</p> Methods <p>Using head-mounted displays, AR materials such as written instructions, surgical videos, schemas, and a 3D neck model were overlaid during a standardized nine-step lateral neck dissection on Thiel-embalmed CST. The questionnaire employed a 5-point Likert scale to evaluate perceived usefulness, usability, and acceptability administered immediately post-session. Feasibility included session duration, biohazard-related incidents, and per-participant cost.</p> Results <p>Fourteen eligible participants completed the full questionnaire, and the response rate was 100%. Overall perceived usefulness was 4.79 ± 0.43, and the perceived added value of AR over conventional CST was 5.00  ± 0.00. The ease of content manipulation scored 4.07 ± 0.92, and the viewing burden through the HMD was 1.57 ± 0.76. Training sessions lasted 78 ± 27&#xa0;min with no biohazard-related incidents. The per-session cost was US$4000, corresponding to US$267 per participant.</p> Conclusions <p>Integrating AR with CST was feasible in our setting and well received, with high perceived usefulness and manageable session-level costs. This approach demonstrates promising utility for supporting self-regulated surgical training, but further controlled studies are required to determine its comparative educational effectiveness.</p>
