<p>Chronic pelvic pain syndromes (CPPS) are based on complex, often combined pain mechanisms. Neuropathic components, particularly peripheral compression neuropathies, such as pudendal neuralgia, are highly relevant in clinical practice. These conditions are primarily diagnosed clinically using the Nantes criteria. Therapeutic intervention requires interdisciplinary, multimodal management. In this context, specialized physical therapy in gynecology, obstetrics, urology and proctology (GUP)&#xa0;represents an essential first-line treatment. It identifies neuronal dysfunction through differentiated somatosensory and neurodynamic assessments and treats it using targeted myofascial decompression, neuromodulation, and systemic approaches within a&#xa0;biopsychosocial context.</p>
