Postoperative pain is often primarily considered nociceptive, although a relevant proportion involves neuropathic components that frequently remain unrecognized. This article outlines pathophysiology, epidemiology, diagnosis, and treatment. Peripheral and central sensitization, nerve injury, and biopsychosocial factors play a key role. Total knee arthroplasty serves as a clinical example to illustrate relevance. Structured assessment and a multimodal treatment approach are essential to prevent chronicity and improve functional outcomes.
