Phantom pain after below-the-knee amputation in a 79-year-old man with type 2 diabetes mellitus and peripheral arterial occlusive disease leads to night-time pain spikes, withdrawal, and uncertainty. In the geriatric setting of innovative age medicine and care, a multimodal, guideline-oriented concept is implemented under the leading involvement of an Advanced Practice Nurse (APN) and a certified expert in pain management. This approach aims to reduce phantom pain, enhance adherence, and support the return to a rural multi-generational household.
