<p>The 2025 European Resuscitation Council (ERC) guidelines redefine resuscitation as an integrated, patient-centred system rather than a&#xa0;sequence of isolated algorithms. In addition to basic and advanced life support, greater emphasis is placed on system organisation, registry data, structured post-resuscitation care, ethical decision-making and the long-term outcomes of survivors. This article outlines the overarching system-based concept of the 2025 guidelines and contextualises the key updates for clinical practice in both pre-hospital and in-hospital settings. It highlights how the individual guideline chapters interact to support outcome-oriented care across the entire chain of survival.</p>
