Innovations in Functional Materials and Advanced Imaging Techniques for Targeting Extramural Venous Invasion (EMVI) in Colorectal Cancer: A Comprehensive Review

Extramural venous invasion (EMVI) is a significant prognostic factor in colorectal cancer (CRC), impacting staging, treatment decisions, and patient outcomes. Recent advancements in imaging, histopathological techniques, and functional materials show promise in enhancing the detection, characterization, and therapeutic management of EMVI. This review aims to critically examine the role of advanced imaging modalities such as diffusion-weighted imaging (DWI) and radiomics in EMVI detection, and to explore the potential of functional nanomaterials for improving diagnostic precision and therapeutic targeting. Advanced imaging techniques, including DWI and radiomics, offer significant promise in EMVI detection, though challenges related to their clinical applicability and limitations remain. Standardization of elastin stain protocols in histopathology and the integration of artificial intelligence (AI) can further improve diagnostic accuracy. Functional nanomaterials like PLGA-based nanoparticles, magnetic nanoparticles, and graphene platforms have shown potential to enhance EMVI detection and facilitate targeted therapy. However, gaps persist in validating these technologies and understanding their relationship with tumor biology. While progress has been made in enhancing EMVI detection and therapeutic management, there is a need for standardized methods and comprehensive validation of emerging technologies. The integration of imaging and molecular data to develop predictive models for personalized treatment is essential, but further research is necessary to establish their clinical relevance. Addressing these challenges could improve EMVI detection and management, leading to better patient outcomes. The insights gained from these innovations could also inform the development of similar strategies for other malignancies, ultimately paving the way for more precise and effective oncologic care.
