Merkel cell carcinoma (MCC) is a rare type of skin cancer. It is an aggressive neuroendocrine carcinoma with a high tendency for local recurrence, lymphatic and distant metastasis. A 76-year-old Caucasian male was referred to our hospital with an MCC of the posterior surface of his left thigh. The patient underwent a preoperative evaluation, during which a mass of the head of the pancreas was detected. An endoscopic ultrasound described a solid mass the biopsy of which confirmed an MCC metastasis. There have been only a few previous case reports of pancreatic metastasis from MCC.
