From host genome to the gut microbiome: an Indian lens on the global diabetes genomics landscape

Diabetes mellitus represents a formidable global health challenge, with the Indian subcontinent emerging as a critical epicenter of this escalating pandemic. The diabetes epidemic in India is distinguished by a unique confluence of genetic predispositions, metabolic characteristics, and a rapid, unprecedented socio-environmental transition, demanding a departure from generalized global research paradigms. This perspective navigates the complex landscape of modern diabetes genomics research, providing a comprehensive overview of foundational domains, including genomics, epigenomics, and metagenomics. By synthesizing landmark global discoveries with the growing body of Indian research, we delineate the current state of knowledge and identify critical gaps that obscure a complete understanding of the disease trajectory in this population. Ultimately, this review argues for a strategic reorientation towards indigenous, population-specific research. We posit that building this capacity is a national imperative, essential for elucidating the unique molecular underpinnings of diabetes in India and for developing accessible, scalable, and personalized healthcare solutions for the diabetes epidemic, the country is currently undergoing.
