Background <p>Women remain underrepresented in surgical journal editorial leadership. Orthopaedic surgery is among the most male-dominated specialties globally, yet no comprehensive analysis has evaluated gender representation across all Web of Science (WoS)-indexed orthopaedic journals. We conducted the first census-level gender analysis of all orthopaedic journals in the WoS Journal Citation Reports (JCR) 2024.</p> Methods <p>All 139 journals in the WoS JCR 2024 “Orthopedics” category were included. Editorial board data were collected from each journal’s official website (January–March 2026). Gender was determined using names, photographs, pronouns, and Genderize.io. Predictors of gender disparity were identified using Chi-square tests and multivariable logistic regression.</p> Results <p>Among 2,325 editorial roles, only 526 (22.6%) were held by women: 10.7% (18/169) among editors-in-chief (EIC) and 23.6% (508/2,156) among board members. Only 15 journals (10.8%) had at least one female EIC; no Asian-based journal had a female EIC. Only 8 journals (5.8%) achieved majority-female editorial boards. Female representation was highest in Q1 journals (33.8% vs. Q4: 6.5%, <i>p</i> &lt; 0.001), North American journals (34.8% vs. Asian: 9.9%, <i>p</i> &lt; 0.001), and society-affiliated journals (24.5% vs. independent: 17.8%, <i>p</i> = 0.003). Defining disparity as female board representation below the 30% critical-mass threshold, 98 of 139 journals (70.5%) fell short. Independent predictors of disparity were Asian journal location (all 30 Asian journals affected; Firth OR: 25.12, <i>p</i> &lt; 0.001), absence of a female EIC (OR: 3.59, 95% CI: 1.16–12.14; <i>p</i> = 0.026), and lower JIF quartile (Q2–Q4 vs. Q1: OR: 3.48, 95% CI: 1.42–8.88; <i>p</i> = 0.006).</p> Conclusion <p>Women hold only 22.6% of orthopaedic editorial roles and 10.7% of EIC positions, placing orthopaedics among the lowest surgical specialties. The complete absence of female EICs in Asian journals and the cascade effect of female editorial leadership demand urgent, coordinated action from journals, publishers, and professional societies.</p>

Gender Disparities in the Editorial Boards of Orthopaedic Journals Indexed in the Web of Science: A Comprehensive Cross-Sectional Analysis of All 139 Journals

Background
