An injury involvingthe area of the meniscotibial and meniscocapsular ligaments in the posterior horn of the medial meniscus is termed a ramp lesion. They usually occur in combination with anterior cruciate ligament (ACL) rupture. Since the meniscal ramp supports the ACL in preventing anterior tibial translation and rotation, repair of unstable ramp lesions is recommended in cases with combined injuries. Thus, the posteromedial compartment should be visualized in every ACL reconstruction and the meniscotibial and meniscocapsular ligaments can be checked with a puncture cannula. If there is doubt regarding the diagnosis of a ramp lesion, it should be directly visualized through a posteromedial portal. Various therapeutic options are available: 1. abrasion, 2. suture with an “all inside” implant via the anteromedial portal, and 3. suture hook repair via a posteromedial portal.
