Die infizierte periprothetische Fraktur

The implantation of a total knee prosthesis (TKP) is one of the most successful orthopedic surgical procedures. Despite higher standards and excellent outcomes 17,456 knee replacements required revision in Germany in 2024, according to the German Endoprosthesis Register (EPRD) annual report 2025. This corresponds to approximately 10.1% of TKPs. The main causes, besides aseptic loosening and ligament instability, include periprosthetic infections (15.2%) and periprosthetic fractures (approximately 3.6%). Each of these complications itself already presents a particular challenge for both the surgeon and the patient. The combination of two complications, such as an infected periprosthetic fracture, is a rare but all the more significant challenge. It requires a well-thought out treatment strategy, knowledge in treatment of both complications combined with personnel and structural provision. The fracture must be stabilized to create the most stable weight-bearing situation possible for the patient and the periprosthetic infection must also be effectively treated. This article aims to provide an overview of possible treatment strategies for infected periprosthetic fractures. Our own data/cases are compared with those found in the literature and evaluated.
