The growing use of artificial intelligence (AI) in medicine requires high-quality, standardized clinical routine data as a foundation for both training and inference. In orthopedics, the potential for data-driven applications is also rapidly increasing, ranging from image analysis and surgical planning to personalized therapy. The German Medical Informatics Initiative (MII) is establishing a nationwide, interoperable research data infrastructure, with Data Integration Centers (DIC) at all university hospitals as its core. These centers extract data from heterogeneous clinical source systems, harmonize them according to a standardized Core Data Set (CDS), and make them available for research in a pseudonymized form. Governance structures, a national Broad Consent, and data protection concepts ensure legal and ethical compliance. The data access process can be initiated centrally via the German Health Research Data Portal (FDPG), which supports both central and federated analyses, and is coordinated locally by Data Use & Access Committees. Numerous application and data utilization projects demonstrate the MII’s capabilities, including in AI-assisted use cases. For orthopedics, opportunities arise in two areas: in the short term, by leveraging existing processes for projects on care quality and outcome research; and in the long term, by defining a dedicated Core Data Set, actively providing data, and integrating with registries. The MII thus offers a powerful platform for local, national, and international projects, as well as a robust foundation for the development and implementation of clinical AI and decision support systems in orthopedics.
