<p>Varus deformity of the limb axis is a&#xa0;significant risk factor for the development of medial knee osteoarthritis. One established treatment option is open-wedge high tibial osteotomy (OWHTO), which corrects the limb axis and reduces medial knee-joint loading. In this case study, the correction of a&#xa0;varus deformity from 5.4° to&#xa0;0.6° in a&#xa0;42-year-old patient with medial cartilage damage was achieved through OWHTO. Although the correction of the limb axis was successful, 3 months postoperatively, normalization of the knee abduction moment was observed, but not of the internal joint moments in the sagittal plane. The instrumented gait analysis allows precise measurement of dynamic joint loading and in the future could help to identify patients who benefit from moderate overcorrection. Preoperative analyses and continuous postoperative monitoring are necessary to assess the long-term outcomes of OWHTO and its impact on joint loading.</p>
