The lateral meniscus plays a pivotal role for knee stability and maintaining knee function throughout life. Treating patients who have undergone total or full segmental lateral meniscectomy is challenging, especially for young and active individuals. In general, the efficacy of meniscus allograft transplantation (MAT) is now widely recognized as being an excellent procedure leading to pain relief and functional restoration. Lateral MAT can be performed using different surgical procedures, but which one is the most effective and safe is still debatable. This article presents a novel variant of arthroscopically assisted lateral MAT combining transtibial root fixation, meniscotibial ligament reconstruction, and soft tissue fixation. For this purpose a four-point fixation is used to restore the anatomy and biomechanics of the meniscotibial junction. These four points are located at the anterior and posterior root and posterolateral and anterolateral point. A short description of the postoperative rehabilitation protocol is outlined.
