To date, scientific publications on cruciate ligament surgery have increasingly focused on the male gender or avoided a dichotomous presentation of results. The growing attention to female professional sports with a noticeably high number of female anterior cruciate ligament (ACL) ruptures now raises the question of underlying causes.
AimAttribution of causes of a gender-specific discrepancy in the incidence of ACL ruptures. Presentation of differences in outcome after ACL replacement and its effects on therapy.
Materials and methodsLiterature search for previous publications; their context and research approaches are then discussed.
ResultsFemale athletes show a 4–8 times higher risk of an ACL rupture. Previous explanations have focused on intrinsic and extrinsic factors. Men show better clinical outcomes and higher return-to-sport rates after ACL reconstruction. The influence on personalized therapy is an approach for future research.
ConclusionWhile there is now sufficient data on gender-specific differences in ACL injuries in athletes and for injury prevention, there is still incomplete data on gender-specific differences in the treatment of ACL lesions; thus, further research is needed.
