Introduction <p>Urolithiasis, a prevalent condition worldwide, poses significant treatment challenges, leading many individuals, particularly in resource-limited settings, to rely on traditional medicines. This study investigated the ethnobotanical knowledge and practices related to medicinal plant use for urolithiasis management in Northwest Algeria.</p> Material and methods <p>A cross-sectional survey was conducted in Tlemcen province, Northwest Algeria, involving participants who met the inclusion criteria (age ≥ 18 years with self-reported urolithiasis). Data were collected through a semi-structured questionnaire and analyzed using descriptive statistics and thematic analysis.</p> Results and discussion <p>The study documented 32 medicinal plant species belonging to 20 botanical families, traditionally used for treating urolithiasis. The most frequently cited plants included <i>Hordeum vulgare</i> L., <i>Herniaria hirsuta</i> L., <i>Citrus limon</i> L., <i>Punica granatum</i> L., and <i>Petroselinum crispum</i> (Mill.) Fuss. The most commonly used parts of the plant were the leaves, followed by the whole plant, and they were most often prepared by decoction and infusion.</p> Conclusion <p>This study underscores the rich ethnobotanical knowledge surrounding urolithiasis management in Northwest Algeria. Further research is crucial to validate the efficacy and safety of the identified plants, which may lead to the development of novel therapeutic strategies. Moreover, documenting and preserving this traditional knowledge is essential for cultural heritage and potential contributions to healthcare innovation.</p>
