<p>Recent advances in medical informatics have demonstrated that information and communication technologies (ICT) can bridge the gap between mental and physical health services, promoting holistic, patient-centred care. This study investigates how integrated digital health systems influence clinical outcomes and patient engagement among individuals with co-occurring mental and physical health conditions in a multi-centre healthcare network. A mixed-methods design combined electronic health record (EHR) analytics with patient-reported experience measures (PREMs) collected through a digital platform. Statistical analysis using multivariate regression explored associations between ICT-mediated interventions—such as tele-psychiatry, remote monitoring, and digital self-care tools—and patient outcomes including adherence, quality of life, and symptom improvement. Results revealed that integrated ICT systems significantly improved care coordination (<i>p</i> &lt; 0.01), enhanced medication adherence by 18%, and reduced emergency admissions by 12% over six months. Qualitative data highlighted improved patient empowerment and clinician–patient communication. The findings suggest that when mental and physical health data streams are interoperable within EHR systems, measurable gains in patient outcomes follow. The study underscores the need for informatics-driven frameworks that unify care delivery, optimize provider workflows, and enhance recovery trajectories across diverse patient populations.</p>
